Cagrilintide reconstitution calculator

A 5 mg Cagrilintide vial with 2 ml of bacteriostatic water holds 2,500 mcg per ml - so 1,200 mcg, as pure arithmetic, is a 48-unit draw on a U-100 insulin syringe. Change any of those numbers and the answer moves; the calculator below does the moving for you, and further down is the thing the vendor calculators never mention: where Cagrilintide actually stands with regulators right now.

Math on the numbers you enter - nothing more. Peptlas sells nothing and suggests no amounts.

Enter your intended dose above and the draw appears here, with the math shown.

Results are arithmetic, not advice. Peptlas is a regulatory-status reference: it does not recommend amounts, schedules, or use of any compound. Read the full disclaimer.

What you are working with

Cagrilintide has no approved standalone form anywhere - Novo Nordisk’s amylin analog exists in clinical trials and in the CagriSema combination (with semaglutide) now under FDA review, neither of which is sold as a vial. What gray-market sellers offer is lyophilized powder, commonly labeled 5 mg or 10 mg, produced outside any quality system - an FDA warning letter names cagrilintide among unapproved drugs sold online, some under coded product names.

The reference table below shows the concentration each common vial-and-water combination produces. These are arithmetic facts about Cagrilintide vial sizes, not guidance about amounts - the same table exists for BPC-157 and TB-500 (Thymosin Beta-4), with different numbers.

Concentration by vial size and water volume for Cagrilintide
Vial+ 1 ml water+ 2 ml water
5 mg5,000 mcg/ml2,500 mcg/ml
10 mg10,000 mcg/ml5,000 mcg/ml

Restricted

Cagrilintide is an investigational drug - Novo Nordisk’s long-acting amylin analog - with no approved form anywhere in the world. It is furthest along as half of CagriSema, a fixed-dose weekly combination with semaglutide: Novo Nordisk submitted a New Drug Application to the FDA on December 18, 2025 on the strength of the Phase 3 REDEFINE trials, and on September 30, 2026 Novo Nordisk said a decision is expected in the fourth quarter of 2026. Until that decision lands there is no legal commercial product containing cagrilintide, and compounding is not an option either: cagrilintide is not a component of any approved drug and is not an eligible bulk substance, so the FDA states plainly that it cannot be used in compounding.

Restricted: Prescription-only, banned, controlled, or under active FDA enforcement. Verified Oct 9, 2026 (ET) · status held since Dec 18, 2025.

Cagrilintide math, asked and answered

How much bacteriostatic water do you add to a 5 mg Cagrilintide vial?
Any amount - water sets the concentration, not the contents. With 2 ml, a 5 mg vial gives 2,500 mcg/ml; with 1 ml it gives 5,000 mcg/ml. More water means the same dose is a larger, easier-to-read draw on the syringe. There is no single correct volume, which is why this page calculates instead of prescribing.
How many insulin-syringe units is 1,200 mcg of Cagrilintide?
It depends entirely on how the vial was reconstituted. As arithmetic: in a 5 mg vial with 2 ml of water (2,500 mcg/ml), 1,200 mcg is 48 units on a U-100 syringe. With 1 ml of water instead, the same 1,200 mcg is 24 units. The calculator above does this for your actual numbers.
